Fine-tuning the leg

Leg alignment and adjustment, when the leg doesn't feel right

Small changes in the angle and position of the foot, knee and socket make a big difference to how a leg feels. If you limp, lean, catch your toe or have new pain in the knee, hip or back, the cause is often alignment, and it can usually be put right while you walk.

In short

What is prosthetic alignment?

The short answer

Every modular leg has adjusters, usually small screws in pyramid adapters, that set the angle of the socket, the knee and the foot. Bench alignment sets them roughly when the leg is built; dynamic alignment fine-tunes them while you walk. If the foot points slightly the wrong way, the socket tilts or the leg is a little long or short, you feel it as a limp, rubbing or pain. Adjusting it takes skill and patience, in small steps, with you walking between each change.

Alignment is checked at follow-ups for legs we make, and for anything else we tell you the cost before any work starts.

When to come in

When an alignment check helps

Ask for an alignment check if

  • You limp more than you used to
  • You have new knee, hip or back pain
  • You catch your toe, or the knee feels unsafe
  • You changed shoes and the leg feels different

It may be something else if

  • The socket is too loose or too tight; alignment cannot fix fit
  • The stump has changed shape; a new socket may be due
  • Weak hip muscles; exercises are part of the answer
  • A worn foot or knee; repair comes first

Your visits

How an alignment check works

  1. Watch

We watch you walk from the front and side, and listen to what you feel.

  2. Check

We rule out socket fit, skin problems and worn parts.

  3. Adjust

Angles and positions changed in small steps.

  4. Confirm

You walk again until it looks and feels right.

Why alignment matters

The socket, knee and foot have to line up with your body so that your weight passes through the leg in the right place at each moment of the step. A few degrees out, and the leg pushes your knee back, tips you sideways or makes the foot slap down. Your body compensates, and over weeks that compensation becomes pain in the stump, the knee, the hip or the back.

Shoes and heel height

The single most common everyday cause of a leg that suddenly feels wrong is a change of shoes. The foot is aligned for one heel height; switch from joggers to formal shoes or chappals and the whole leg tips forward or back. Tell us what you wear most, and bring the new shoes when you come; re-setting the foot angle is quick.

Knee, hip and back pain

A leg that is even a little too long makes you hitch your hip or swing the leg out; one that is too short makes you dip. Both strain the back. An alignment that keeps the knee pushed back can hurt the knee on the artificial side, and favouring the leg overloads the other knee and hip. Many of these pains ease once the leg is set right and you are walking evenly again.

Why you should not adjust it yourself

The adjustment screws look simple, but a small turn changes the angle of the whole leg, and two screws affect each other. Well-meant adjustments at home are a common cause of pain and falls. If the leg feels wrong, stop adjusting and call us; a short visit is safer and usually quick.

Getting an alignment check from another city or country

Tell us your city or country and your travel dates when you call. We plan the visits as close together as the stump allows, so most patients can finish the casting, test socket and fitting in one stay, whether they come from Quetta or fly in from Dubai, with a shorter trip later for adjustments if needed.

Questions

Questions about the leg alignment and adjustment

Why does my artificial leg hurt my back?

Often the leg is a little too long or too short, or set at an angle that makes you lean. An alignment check usually finds and fixes it.

I changed my shoes and now I limp. Why?

The foot is set for one heel height. A higher or lower heel tips the whole leg forward or back. Bring the shoes and we will re-set the foot.

Can I adjust the screws myself?

Please do not. Small turns of those screws change the angle of the whole leg and can cause pain or falls. Call us instead.

How much does an alignment adjustment cost?

Alignment is checked at follow-ups for legs we make. For anything else, we explain what is needed and tell you the cost before any work.
