Ankle disarticulation

Syme's ankle prosthesis for a long, end-bearing stump

A Syme's amputation removes the foot at the ankle but keeps the heel pad, so you can often stand on the end of the stump. The leg is short and strong, but the stump is long and wider at the end, so the socket and a very low foot have to be designed around it.

In short

What is a Syme's ankle prosthesis?

The short answer

It is the leg for an amputation through the ankle joint, where the heel pad is kept and stitched over the end of the shin bone. Because the heel pad is made for weight-bearing, most people can put weight straight onto the end of the stump, and many can take a few steps at home without the leg. The socket has to pass over the slightly wider end and grip above it, and the foot has to be very low because there is little space under the stump.

Prices range from Rs 110,000 to about Rs 700,000 with an imported low-profile carbon fibre foot.

Is it right for you?

Who a Syme's prosthesis suits

Usually a good fit if

  • The heel pad is well healed and sits firmly on the end
  • You want a strong, end-bearing leg
  • Long stump with healthy skin
  • Active users who walk long distances

We may advise waiting, or something else, if

  • The heel pad has slipped to one side or is painful
  • Ulcers on the end of the stump, more common with diabetes
  • If the heel pad cannot take weight, the socket is shaped to load the shin instead

Your visits

Four visits for most Syme's legs

  1. Assessment

We check the heel pad, the skin and the length, and how much weight the end takes.

  2. Casting

An impression that captures the wide end and the narrower ankle above it.

  3. Test socket

A clear socket shows whether it passes over the end and grips well.

  4. Fitting and training

The low foot is aligned while you walk, and training starts the same day.

The advantage: standing on the end

The heel pad is the tissue made by nature to take your weight, and after a well-healed Syme's amputation it keeps doing that job. Standing feels natural, you feel the ground through the stump, and the long lever makes walking efficient. Many users can take a few steps to the bathroom at night or for wudu without putting the leg on, which reduces the risk of falls.

The challenge: a wide ankle and little room for the foot

The end of the stump is wider than the ankle above it because of the bone ends and the heel pad. The socket has to pass over this bulge, so it has a removable window or a flexible inner wall. The finished ankle looks a little wider than the other ankle, though careful shaping and a cover reduce the difference.

Because the stump is almost the full length of the leg, there are only a few centimetres for the foot. That is why Syme's feet are special low-profile designs, and why the choice is smaller than for below knee legs.

Life with a Syme's leg in Pakistan

Taking the leg off for wudu is quick, and many users can stand briefly on the heel pad while they wash. Closed shoes suit the leg best; the low foot fits a normal shoe without trouble. In summer, dry the stump and the liner at midday, because sweat softens the skin over the heel pad.

Caring for the heel pad

The heel pad is the most valuable part of a Syme's stump. Check it every night for redness or broken skin, avoid walking long distances on it without the leg, and keep the skin moisturised but not soggy. If you have diabetes, any sore on the end needs your doctor the same day.

Questions

Questions about the syme's ankle prosthesis

Can I walk without the leg after a Syme's amputation?

Many people can take short steps at home on the heel pad, which is useful at night or for wudu. For anything longer, wear the leg; walking on the bare stump overloads the heel pad and can damage it.

Why does my artificial ankle look wider than my other ankle?

The end of the stump is naturally wider because of the bone ends and the heel pad, and the socket has to go around it. Careful shaping and a cover reduce the difference, but some extra width usually remains.

How much does a Syme's prosthesis cost?

From Rs 110,000 for a local build to about Rs 700,000 with an imported low-profile carbon fibre foot. Because space is limited, there are fewer feet to choose from than for below knee legs.

Is a Syme's amputation better than below knee?

When the heel pad stays healthy, a Syme's stump gives a long lever and end-bearing that many people like. The decision about the level is your surgeon's. Our job is to make the best leg for whichever level you have.
